That is accurate to how some of them looked in Silent Hill 2. As to what it is, that's not entirely clear. It kinda resembles the save points in 2 but much dirtier. It also kinda looks like a photo of a busted pair of lips or nasty toothless gums placed over where the mouth should be.

There were many designs to the nurses, and that was one of them. If there was ever an opportunity to pack in a few more headsculpts to make people buy more than one, Iconq Studios missed it.
