You forgot Captain Piett, not only did he get field promotions twice by Vader (three, if you go by Expanded U), but was also spared at least once by Vader as well.

I would love to see him made, although I think it would be really cool if they could include accessories so we could alter him to match his promotions like in the OT: Lieutenant >> Captain >> General >> Admiral.
