It kind of depends on whether or not you count 2008's Incredible Hulk as part of Phase 2. There are only a few ties between it and the rest of the films, so it could just as easily be ignored as 2003's Hulk. At least as far as the MCU is concerned.
Actually Increible Hulk is very much part of phase one. You have to imagine everything up to the Avengers was all in a matter of a week to 2 weeks in actual time. The whole reason Blonsky and Banner were both transformed is they (GOV.) was trying to recreate the super solider process. Ross himself say," Banner didnt know he was working on this project, and the dumb bastard tried it on himself". Also at the end after trailer of Hulk, Tony meets with Ross in a bar to change Ross'S mind cause they were considering using Blonsky on the Avengers team. If I remember correctly,The order in real time is Iron Man 1 then IM2,the hulk happen almost at the same time Thor is happening, Then they find Cap and few weeks later The events of the Avengers movie happen.
