That's the American cover art, which is known far and wide as one of the worst videogame covers ever. It was made back when Capcom's U.S. branch was well-known for replacing the Japanese game art when some of the worst crap imaginable (this was before Anime caught on in the States). This is the original Japanese cover:

It's the Anime art style, which is all about exaggerated proportions. Either you like it or you don't, I guess.

Now I have never played this game so I'm coming in totally unbiased. Maybe their video rendering is way better than the art presented here.
Well, check out this playthrough to see what you've been missing out on...AvP is easily one of the best side-scrolling beat-em-ups ever made, in my opinion, and can easily be played on any modern PC via MAME. The dude doing this playthrough is an absolute
master
, beating it on the hardest difficulty without dying ONCE. Your average gamer sure as hell can't do that (I die plenty), but you can tell that this guy has played the game 10,000,000 times and has every enemy pattern completely memorized. But I wanted to show you the sheer gameplay variety and combo possibilities available once you know what you're doing (as opposed to other playthroughs, where people just presses the main attack button over and over and nothing else):
