Well for the figure the only thing under the pyramid is his neck and a fairly nasty looking flesh membrane which you can see some of squeezing out of the back. In Silent Hill 2 he never removes it so I've always figured that it just is his head. He is an evil tormentor demon thing which embodies all of the main character's fears and frustrations so it's not out of the question. The flesh membrane then is either a grotesque display of muscle needed to move such a head or maybe if there is something under the pyramid then it could be a safety bladder (made of flesh to be gross) to assist in wearing the helmet. There is this thing he can do in the game where an "appendage" emerges from that hole in the front which may be indicative of what's underneath but then since he is a monster this could mean anything. In Silent Hill: Homecoming Pyramid Head returns bearing his movie design and is called the "bogeyman" by the children. In Homecoming's true bad ending (for the player being a total jerk) the main character is turned into a Pyramid Head where we see two other "bogeyman" monsters encase his head within two halves of the pyramid with both being filled with spikes on the inside, like an iron maiden exclusively for the head. However since the town of Silent Hill reacts and creates its monsters based on each character this doesn't mean that the Pyramid Head(s) from SH2 share that trait.
