The people who work at Hasbro were not hired to be fans. They don't get to sit around all day reading comic books or watching movies and cartoons. I think they succeeded in making a figure based on the above image. Sure, a couple of extra paint apps wouldn't hurt but, overall, I think they got it. As for the extra head, maybe Marvel wanted to include it or perhaps the Legends team wanted to add something extra. I don't know. But if you look at images on Google, Omega Sentinel doesn't have a consistent look. To be honest, I thought the heads were of two different women who were Omega Sentinels, not the same person. Regardless, it looks like they chose this image to base the alternate head on.





Like Veronica Mars said, who knows what came up in their Google Image search when they designed the alternate head. But like I said above, their job isn't to read up on the character and know her back story. It's to create an action figure based on 2D artwork. Marvel clearly felt the same way because they approved the figure's design.
I'm glad you posted these pictures because it shows straight up how the Hasbro team neglected to color her correctly. The image of her in the mask clearly shows both significantly darker hair and noticeably darker skin. If this was the image they used as reference, they used it poorly.
