No, that was HHH setting Kane up. HHH only accused Kane of doing it and lied about having proof. It was also part of one of the most idiotic and least popular story arcs WWF/E ever did. IMHO.

I still believe HHH. Kane looks like the type to me. Besides, I see no reason to doubt HHH, he seems like a pretty honest guy; it isn't like he put Rikkishi up to running Austin over with a car or was responsible for smashing HBK's face through a car window or anything.

Yes, though they only have the same mother. Kane is Bearer's son according to a paternity test they took. Apparently Undertaker's mother was a big slut.
