It's just normal Blade form.

For blade he has more or less 3 forms.
Blade - Baseline armor
Fusion Jack/Queen = Jack Form
and King Form.

This is just a baseline form. Unless it has the rouser and wings, then it could be baseline +jack form.
King form is tons of gold trim, and gold sigils on each limb. The helmet on king form also has more 'crown like' points.
